What You Should Know About Your First Visit
Thank you for choosing our office for your dental needs. We look forward to getting to know you and your family, and we appreciate your referrals.
Once you receive your welcome packet, please complete, sign and return the forms to our office so we may import them into your electronic record. If you have records you would like transferred, please complete and forward the Records Release Authorization form to your previous Dentist so this information may be emailed to us. Please arrive 15 minutes prior to your appointment time to complete your registration and check in process. If you did not receive a welcome packet, please arrive 20 to 30 minutes prior to your appointment to complete these forms. If you would like help completing these forms, we will be happy to assist! Kindly let us know prior to your appointment so we may arrange a confidential and private setting to gather this information.
All new patients over the age of 16 will meet with one of our Doctors for a comprehensive oral examination. During this visit, we will gather information based on your current dental health, and ask you questions regarding your past treatments. An oral cancer screening will be done at this and each subsequent re-care visit. We will determine your periodontal (bone and gum tissue) health and discuss with you any special consideration to treat any periodontal disease.
Diagnostic aids such as x-rays and photos will also be gathered during this visit. To prevent ghost images on your x-rays, we ask that all jewelry, including necklaces, earrings, and other facial jewelry not be worn to your initial appointment. Using these aids, along with the oral examination, your current needs will be thoroughly discussed, and the Doctors will review with you your diagnosis and treatment options. If your previous dentist, oral surgeon, or other dental specialist has taken x-rays, please request that they be e-mailed to our office. If additional films are necessary, they will be taken at our facility.
Children under the age of 16 are scheduled to see one of our Hygienists. During this visit, your child can expect a full examination with one of the Doctors, x-rays and/or photos for diagnosis, a prophylactic “cleaning”, appropriate fluoride treatment and basic home care instruction. We will review with you your child’s diagnosis, treatment needs and options.
Please bring any insurance information and insurance cards with the subscriber identification information. Also, to insure your privacy, we need a photo ID to scan to your record. A driver’s license, state issued ID card, school or college ID’s are all accepted forms of identification. If these are not available, we are happy to take your picture upon your arrival.
